Job Description:
As a District Nurse with a specialisation in mental health at Burton Town, your primary responsibility will be to provide comprehensive, person-centered mental health care to patients in their own homes. You will play a critical role in supporting individuals with mental health challenges, ensuring they receive the care, education, and support they need to maintain and improve their mental well-being.
Key Responsibilities:
Assessment and Care Planning: Conduct comprehensive assessments of patients' mental health needs and develop personalized care plans that address their specific challenges and goals.
Mental Health Care: Provide expert mental health nursing care, including assessment, intervention, and evaluation, to patients with a range of conditions, such as depression, anxiety, bipolar disorder, and schizophrenia.
Medication Management: Administer and monitor psychiatric medications as prescribed, and educate patients and their families about the proper use and potential side effects.
Crisis Intervention: Be prepared to respond to mental health crises, including providing support during acute episodes and coordinating emergency psychiatric services as necessary.
Collaboration: Collaborate with a multidisciplinary team of healthcare professionals, including psychiatrists, psychologists, social workers, and community services, to ensure holistic care.
Education and Support: Provide education and support to patients and their families to help them better understand and manage mental health conditions.
Documentation: Maintain accurate and detailed patient records in compliance with healthcare regulations and standards.
Health Promotion: Implement mental health promotion initiatives within the community, with a focus on prevention and early intervention.
