Job Summary:

We are seeking a highly skilled and motivated Advanced Clinical Practitioner to join our Urgent Community Response Team in Stoke. The successful candidate will play a crucial role in delivering high-quality urgent care services within the community. As a key member of the team, you will be responsible for making senior clinical decisions, working autonomously, and contributing to the effective functioning of the Urgent Community Response service.

Key Responsibilities:

  1. Senior Clinical Decision Maker:
    • Utilize advanced clinical skills to assess, diagnose, and treat patients with urgent care needs.
    • Make sound clinical decisions, providing leadership and expertise to the Urgent Community Response Team.
    • Collaborate with other healthcare professionals to ensure seamless patient care.
  2. Autonomous Working:
    • Work independently and autonomously within the scope of advanced clinical practice.
    • Take the lead in managing complex cases, providing timely and effective interventions.
  3. Urgent Community Response:
    • Respond promptly to urgent care requests within the community.
    • Conduct home visits and assessments, delivering patient-centered care tailored to individual needs.
  4. V300 Prescriber:
    • Hold a V300 prescribing qualification, demonstrating the ability to prescribe medications within the advanced practitioner role.
    • Prescribe and review medications in line with professional standards and guidelines.
  5. Car Driver:
    • Possess a valid driver's license and have access to a car for transportation.
    • Be willing and able to travel to various locations within Stoke to provide timely responses to urgent care needs.

Qualifications and Experience:

  • Registered as an Advanced Clinical Practitioner with a relevant professional body.
  • Hold a V300 prescribing qualification.
  • Proven experience as a Senior Clinical Decision Maker within urgent care settings.
  • Ability to work independently and autonomously.
  • Excellent communication and interpersonal skills.
