We are currently looking for an experienced advanced nurse practitioner, candidate must be hard working, proactive, and able to work under pressure, we are looking for an expert clinician who is a resource for colleagues within the service and carries the ability to lead the nurse practitioner team strategically and clinically, but most importantly they must always put patient care first.

  • Pay rates will depend on experience but will range from £42 - £55 an hour.
  • Hours of work: MONDAY to FRIDAY 37.5hrs a week.

JOB DESCRIPTION:

Candidate will be an Advance Nurse Practitioner for the Hospitals inpatient service working across 3 wards and also provide clinical support to The Grand short stay reablement unit when required, they will also be providing clinical leadership and senior clinical advice to the ward and unit teams; ensuring that the team provide clinical excellence in the delivery of patient centred care.

EXPECTATIONS:

Candidate will be expected to:

 

  • Use expert theoretical and practical knowledge to receive referrals from or refer to other departments within or outside of the Trust.
  • Expert knowledge of and ability to access appropriate community resources to enable safe discharge planning.
  • Have a high degree of understanding of cognitive problems and the ability to initiate management of cognitive issues into clinical care interventions and care planning.
  • To lead a multidisciplinary approach to the management and prevention of falls for inpatients and in liaison with the community falls prevention teams.
  • Exercise an advanced level of professional autonomy involving complex facts and situations which require analysis and interpretation of medical and non-medical data leading to the implementation of best practise strategies specific to each patient.
  • To work with the ward manager, head of service and discharge facilitator to further develop protocols of care and design patient care pathways.
  • Make clear decisions with confidence and communicate these effectively.
  • Demonstrate an expert ability to communicate highly complex and sensitive information and medical advice regarding conditions, expected outcomes and often with significant barriers to understanding for patients and carers.
  • Effectively articulate the expert perspective on a patient’s condition with members of the multidisciplinary team when various and possibly conflicting management options are available, facilitating the reconciliation of inter professional differences of opinion.
  • Liaise with GP’s and other health care professionals as required advising them on patient outcomes, treatment and follow up.
  • Undertake highly complex and often emotional discussions with patient, families and staff regarding clinical or operational issues.
  • Develop discharge plans that will ensure safe and effective living of older people in the community and the home environment.

SKILLS:

Analytical and Judgement:

  • Candidate is required to make complex decisions and judgements to assess specialist clinical conditions and lead in the development of care and treatment plan.
  • Candidate must be able to interpret blood results/ECG’s/X-Rays and data relating to clinical care management, in order to be able to formulate correct diagnoses and implement adequate care.
  • Candidate must be able to work and make decisions under pressure.
  • Candidate must be able to recognise, detect and lead early intervention of acute life threatening events.

Planning and Organisational:

  • Candidate must be able to organise own workload, by working systematically to achieve optimum patient care.
  • Candidate must be able to plan own working diary in order to meet clinical requirements and meeting attendances.
  • Candidates must be able to prioritise and manage frequent changes of plans and be able to respond to clinical needs of patients and clinical emergency situations.
  • Candidates must be able to plan individual patient care programmed and undertake regular reviews of progress, implementing changes when required.

Physical:

  • Candidate must be able to carry out physical examination of patients to a high calibre.
  • Standard keyboard skills.
  • Candidate must be suitable to assist in the administration of intravenous/intramuscular, and to carry out venepuncture and inserting cannulas.

ESSENTIAL REQUIREMENTS:

  • NMC Registered.
  • Degree in relevant subject area.
  • MSc in advanced clinical practice/relevant subject area or working towards/equivalent knowledge and skill.
  • Significant experience in managing the care of an older person.
  • Teaching qualification/experience.
  • Portfolio of lifelong learning.
  • Independent Non-Medical Prescriber.
  • IT skills.
  • Excellent clinical skills.
  • Ability to manage their own caseloads.

DESIRABLE KNOWLEDGE:

  • Rehabilitation qualification.
  • Evidence of research/publication.
  • Advanced life support.
  • Clinically relevant course/skills.
  • Mental Health knowledge.
