Responsibilities:
- Coordinate and oversee the community nursing service for a defined area, ensuring timely intervention and face-to-face visits
- Provide clinical leadership, advice, and support to Palliative Care Nurse Practitioners and other staff members
- Participate in multi-professional meetings and advocate for patients, representing nursing
- Undertake patient assessments and advanced-level care, including nurse prescribing
- Collaborate with interdisciplinary teams and ensure patients' physical, psychological, social, and spiritual needs are met
- Contribute to service development initiatives and maintain clinical governance standards
- Actively engage in ongoing education and training opportunities
- Foster positive relationships with healthcare professionals and promote hospice services
Requirements:
- Registered Nurse with the appropriate licensing authority
- Experience in community palliative care or related field
- Strong leadership and managerial skills
- Excellent clinical assessment and care coordination abilities
- Commitment to continuous improvement and evidence-based practice
- Effective communication and collaboration skills
- Passion for providing holistic palliative care and supporting patients and families
