Using a pro active and skilled approach to access physcial, sensory, cognitive, communicative and behavioural impairments following illness and/or injury

Using a proactive and skilled approach to provide rehabilitation plan to enable clients to reach there maximum potential in achieving their daily living activities

To work Collaboratively with other medical practitioners, therapists, case managers, statutory authorities, legal representatives, indepenedent organisations and employers to facilitate a return to daily activities within their home enviroment & community

To deliver the service in a timely fashion, following directionfrom instructing solicitors, case managers and funders where relevant

Key responsibilities:

To undertake comprehensive assemssment of abilities, noting and impairment.

To make reccomendations to the appropriate agencies e.g statutory services, voluntary organisations & independent providers

To implement an agreed rehabilitation programme, regularly reporting the clinets process

To Liase with statutory service providers and negotiate apporpriate provisions in repsect of support for daily living activities, equipment and accomodation adaptations where applicable

To liase with the instructing solicitor/case manager to secure funding to meet need

To liase with care agencies and participate in the recruitment of support workers and to provide appropiate to the needs of the client

To educate/train family carers as required
